As part of Olmsted Medical Center’s Active Aging Services team, the Nursing Home Services team delivers compassionate, expert care directly to residents of skilled nursing facilities in southeastern Minnesota. By bringing care to where residents live, we support their health, independence, and overall quality of life.

Comprehensive Care

Our clinicians provide both chronic and acute medical care, as well as mental health support, to help residents stay well and avoid unnecessary hospital visits. Services include:

regular assessments and treatment for ongoing health needs
medical directorship for skilled nursing facilities
care planning and coordination with residents, families, and facility staff
support with admissions and discharges
education for nursing facility staff on best practices in resident care.
