Stay healthy, independent, and connected with Olmsted Medical Center’s Active Aging Services (AAS). We offer compassionate, team-based care and community resources designed for adults age 55 and better. Our goal is to help you stay safe, active, and engaged in the life you want to lead.

What We Do

AAS helps you navigate health care, manage chronic conditions, prevent falls, and connect with local partners and support services. We provide:

  • Geriatric consultation and care planning
  • Memory and cognitive evaluations (memory consults)
  • Palliative care and symptom support
  • Falls prevention assessments and home safety planning
  • Medical cannabis consultation (when appropriate)
  • Care coordination with your primary care provider and specialists
  • Community programs—Tai Ji Quan balance classes, caregiver support groups, PEARLS behavioral coaching, and more

We work with family members, caregivers, and community partners to create a plan that fits your goals and daily life.

Age-Related Conditions We Support

We help assess and manage common concerns that affect older adults, including:

  • Memory loss and dementia
  • Balance problems and falls risk
  • Chronic disease management (diabetes, heart disease, COPD, arthritis)
  • Mobility and strength issues
  • Medication review and safety
  • Pain and symptom control
  • Social isolation, caregiver stress, and behavioral health

Preventive Care & Screenings

Staying well starts with prevention. AAS helps you stay up to date and connected to services such as:

  • Annual wellness visit planning
  • Vision and hearing screening referrals
  • Cancer screening reminders (mammography, colonoscopy)
  • Vaccinations (flu, pneumonia, shingles, Tdap)
  • Cognitive and fall-risk screening
  • Medication reviews and deprescribing when appropriate

Chronic Disease Management & Mobility Support

We coordinate care to help you manage long-term health conditions and stay active:

  • Medication management and reconciliation
  • Exercise and mobility plans (including referrals to physical and occupational therapy)
  • Rehabilitation and assistive device recommendations
  • Nutrition counseling and diabetes support
  • Home health and infusion care coordination

Memory, Thinking, and Mood Support 

If you or a loved one notices memory changes, AAS offers:

  • Cognitive testing and memory consults
  • Thorough medication reviews to identify contributing factors
  • Diagnosis and staging of mild cognitive impairment and dementia
  • Care planning, safety recommendations, and community resources for caregivers
  • Referral to specialists and dementia units when needed

Falls Prevention

Falls are preventable. Our falls program includes:

  • Home safety assessments and education
  • Strength and balance testing (Tai Ji Quan and exercise referrals)
  • Medication review to reduce fall risk
  • Coordination with local partners for home modifications and in-home support

Community Programs & Evidence-Based Classes 

We offer and refer to proven programs that improve strength, mood, and caregiver skills:

  • Tai Ji Quan — balance and mobility training to reduce falls
  • Powerful Tools for Caregivers — six-week class for caregiver skills and self-care
  • PEARLS — short-term coaching for depression and activity engagement in older adults
  • Education on advance care planning and advance directives

How to Get Care & Referrals 

Getting started is simple.

  • Ask your primary care provider for a referral to Active Aging Services, or
  • Call AAS directly at 507.287.2794, Monday–Friday, 7:00 AM–5:00 PM (leave a message after hours)
  • Send a message through OMC MyChart to request a consult

We’ll review your needs, explain next steps, and schedule an appointment or home visit as appropriate.

Who You'll Work With

AAS is a multidisciplinary team including geriatric-trained clinicians, nurses, social workers, physical therapists, and community health workers. We coordinate with specialists across OMC and community partners to offer the care you need in the most convenient setting—clinic, home, or virtually.

Need community help now? AAS connects you with local services for transportation, home safety, respite care, food support, and financial assistance. We partner with agencies including home health, Veterans Services, faith-based groups, and local housing programs to find the right fit for you.

We help you navigate payment and logistics:

  • Information about Medicare annual wellness visits and coverage
  • Help accessing prescription delivery, bubble packs, and Medication Therapy Management (MTM) through OMC Pharmacy
  • Financial assistance referrals and social work support for qualifying patients
