What to Expect at Your Well Child Check Starting January 2025

Olmsted Medical Center is enhancing Well Child Checks with these updates to improve care for your child:

Pre-visit Questionnaires

  • These forms will be sent to families via MyChart one week before the appointment. Completing them in advance ensures your care team has everything they need to prepare for your visit, helping make the most of your time with your provider.

Confidential Teen Surveys

  • Adolescents aged 11+ will complete a confidential survey on paper during their visit. This survey allows for open, private discussions about sensitive health topics. Only the provider will review the responses, and they won’t be included in the medical chart.

Parent/Teen Confidentiality Letter

  • Families of adolescent patients will receive a letter explaining the updates and providing information about adolescent privacy laws related to sexual health and substance use disorder care.

We're committed to ongoing improvement and value your feedback. For more information, visit www.olmmed.org
