  • Call ahead to schedule your appointment — this helps minimize your waiting time.
  • Share all topics you’d like to discuss when booking, so we can schedule enough time. If you have multiple concerns and don’t mention them in advance, we may need to set up a follow-up visit.
  • Bring a list of your current medications, including herbal and non-prescription supplements. If possible, bring them in their original containers.
  • Bring a copy of any advance directives, such as a living will or organ/tissue donation form, so we can add them to your medical record.
  • Arrive by your assigned check-in time (a few minutes early is even better).
  • Co-pays are due at the time of your visit.
