Submitting Insurance Information
To help us file your insurance claims correctly, please provide your current insurance card or coverage information at the time of registration.
As a courtesy, OMC will submit your claim to your insurance carrier on your behalf. You are still responsible for:
- Confirming your coverage and benefits with your insurance provider
- Following up with your insurance company if payment has not been made within 45 days
- Paying any remaining balance after insurance has processed your claim
If your account remains unpaid after 60 days and no payment arrangements are made, your balance may be billed directly to you or referred to a collection agency.

If you have questions about your coverage, benefits, or payment status, please contact your insurance provider directly. The customer service phone number is usually found on the back of your insurance card.
